Assumption of Risk & Liability Waiver

I understand that participation in volleyball and related physical activities involves inherent risks, including but not limited to injury, illness, or accidents.

On behalf of my child, I voluntarily assume all risks associated with participation in this camp and agree that Together We Move, its organizers, coaches, staff, and MSA Woodland are not liable for any injuries, damages, or losses that may occur as a result of participation.

I certify that my child is physically able to participate in camp activities.

Medical Authorization

In the event of an emergency, I authorize Together We Move staff to seek medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

Photo & Media Release

I grant permission for my child to be photographed or recorded during the camp. I authorize Together We Move to use these images or videos for promotional, marketing, and social media purposes without compensation.

No Refund Policy

I understand that all registrations are final and that no refunds will be issued for any reason, including scheduling conflicts, illness, or inability to attend.

Consent & Agreement

By signing below, I acknowledge that I have read and agree to the terms of this waiver on behalf of my child.
